Perioral dermatitis also referred to as perioral eczema is a skin condition that reveals itself by a red rash on the skin around the mouth. It looks like acne and thus many people confuse it with acne but it is a different skin condition.
One can experience dryness, scaling, and flaking of the skin around the mouth with swollen, inflamed bumps on the skin called papules. For some people, this kind of rash is accompanied by itching or burning sensation.
The cause of perioral dermatitis is not certain, but it is associated with the use of topical steroids, face creams, and other skin products.
One should note that perioral dermatitis cannot be transmitted from one person to another, so there is no need to worry about it being contagious.
Symptoms of Perioral Dermatitis
- Red rash around the mouth
- Scaly, dry, and flaky skin
- Swollen, inflamed bumps (papules)
- Itching or burning sensation
- Rash can spread to the nose and eyes
- Sometimes presents as clear fluid-filled bumps (vesicles) or white fluid-filled bumps (pustules)

Perioral Dermatitis Treatment
- Stop using topical steroids: This is usually the initial treatment that is given to patients with perioral dermatitis. Avoid using any corticosteroids; this includes hydrocortisone creams.
- Avoid heavy creams and moisturizers: Use gentle, unscented products that are less likely to cause reactions on your skin.
- Change toothpaste: Use non-fluorinated toothpaste to prevent potential irritation.
- Topical medications: Options include erythromycin gel, clindamycin lotion or gel, metronidazole cream or gel, pimecrolimus, azelaic acid, and sulfur preparations.
- Oral antibiotics: Medications such as tetracycline, doxycycline, minocycline, or oral erythromycin may be prescribed to help reduce inflammation and treat any bacterial involvement.
- Skin care routine: Use a mild, fragrance-free cleanser and be very gentle when washing your face. Avoid using harsh scrubs or irritating products.
- Lifestyle changes: Identify and avoid triggers such as certain foods, stress, and environmental factors that may aggravate your condition.
